Application Instructions

  1. Applicants should present the following documents to the Admissions Office:
    1. Applicants who have started their college studies should present their Official Transcript of Records, NSO Birth Certificate and curriculum vitae or résumé.
    2. Applicants who are high school graduates should present their High School Report Card or PEPT/NFE-ALS certificate, NSO Birth Certificate and curriculum vitae or résumé.
  2. Schedule initial interview with the Registrar. The Registrar will evaluate Transcript of Records and list the credited subjects in the “Evaluation of Credits Form.”
  3. Schedule interview with the Chairperson of the degree program applied for. Present the “Evaluation of Credits Form” to the Chairperson then the Chairperson will issue Advisement Form.
  4. Secure and fill out application slip at the Admissions Office.
  5. Pay the application fee of Php400.00 at the Cashier’s Office.
  6. Go back to the Admissions office to secure the application form.
  7. Submit filled out application form. Admissions Office will issue the Acceptance Letter.
  8. Present Acceptance Letter and Advisement Form to the Registrar’s Office to register advised subjects.
