Application Instructions
- Applicants should present the following documents to the Admissions Office:
- Applicants who have started their college studies should present their Official Transcript of Records, NSO Birth Certificate and curriculum vitae or résumé.
- Applicants who are high school graduates should present their High School Report Card or PEPT/NFE-ALS certificate, NSO Birth Certificate and curriculum vitae or résumé.
- Schedule initial interview with the Registrar. The Registrar will evaluate Transcript
of Records and list the credited subjects in the “Evaluation of Credits Form.”
- Schedule interview with the Chairperson of the degree program applied for. Present
the “Evaluation of Credits Form” to the Chairperson then the Chairperson will issue
Advisement Form.
- Secure and fill out application slip at the Admissions Office.
- Pay the application fee of Php400.00 at the Cashier’s Office.
- Go back to the Admissions office to secure the application form.
- Submit filled out application form. Admissions Office will issue the Acceptance
Letter.
- Present Acceptance Letter and Advisement Form to the Registrar’s Office to register
advised subjects.
